The on-line application must be supported with printed documents which you date, sign, and mail to the Program Office, along with your application fee. Instructions for application and the supporting documents are in PDF files. To view and print the PDF files, you must have Adobe Reader installed on your computer. If you do not have Adobe Reader, you can download a free copy from Adobe. Print out the Application Instructions file. Use those instructions as a guideline and checklist to complete and submit your application. View and print the Description of Program file, which contains information about the program, curriculum, clinical work, the profession, and practice. Print out the Reference Forms and complete your section of each of the three reference forms. Provide one form and a return envelope to each of your three references. Do this as soon as possible because your application will not be complete until all three references have been received. Complete the online application. You may first see a page indicating a problem with the website security certificate. To continue onto the application, please select the second option, "Continue to this website." Leave no section incomplete. Double check the application before submitting. You will need to have copies of all of your transcripts (unofficial or official) in front you to complete some fields in the application. Incomplete applications will not be considered. Obtain a current, full color passport-sized photograph. Do not copy your passport - send an original color photograph, which is readily available from Kinko's or similar business. Send a check or money order payable to Emory University for $60.00 in payment of the nonrefundable application fee. Send all six supporting documents (along with the passport photograph and payment of application fee) to the Program Office. Have transcripts from all graduate and undergraduate institutions in which you have been enrolled mailed to the Program Office. Request transcripts as soon as possible because delay in receipt of transcripts is one of the most common reasons that applications remain incomplete. If you have any prerequisite courses in progress, include proof of enrollment in the courses (e.g., unofficial transcript, letter from the registrar's office). Note: The online, electronic application, along with the supporting printed documents, constitute a complete application for admission – they are not an offer of admission or a contract for enrollment. Qualification criteria specified in the application are the minimum criteria, and no applicant is guaranteed admission to the Program, even though he/she may meet the minimum criteria. All applicants should be aware that gaining admissions to the Emory Anesthesiologist Assistant Program is highly competitive.
