Re-application Process If you applied last year and were declined admission to the program, you are welcome to reapply. The program office will hold your application file for one year. If you choose to re-apply, do not go through the entire application process again. Please submit the following items: View and print the Description of Program file, which contains information about the program, curriculum, clinical work, profession, and practice. Print out two Reference Forms and complete your section of each of the reference forms. Provide one form and a return envelope to each of your two references. Please select individuals different from those who have already completed a reference form for you. Do this as soon as possible because your application will not be complete until both references have been received. Create a new account and submit a new online application or send the program office any updates to last year's application with your supporting documents (new grades or exam scores, change of address, email address or telephone number, etc). Submit a check or money order payable to Emory University for $35.00 in payment of the nonrefundable re-application fee. Submit a personal statement indicating that you wish to reactivate application. Include information about how you have increased your knowledge of the profession, improved test scores, or made improvements to your application (if applicable). Have any new or updated transcripts from all graduate and undergraduate institutions in which you have been enrolled mailed to the Program Office. Request transcripts as soon as possible because delay in receipt of transcripts is one of the most common reasons that applications remain incomplete. Print out, complete, date, and sign any of these supporting documents if you did not do so last year: Documentation of Familiarity with Anesthesia Practice Documentation Concerning Technical Standards Documentation Concerning Infectious and Communicable Diseases Criminal Background Check Process Drug Screen Release Form Applicant Statement
All supporting documents, updated application information, payment of re-application fee, and other items should be mailed to the Program Office:
MMSc Program in Anesthesiology 57 Executive Park South - Suite 300 Atlanta, GA 30329
Note: For your application to be complete, all of the above steps must be completed. Incomplete applications will not be considered. The applicant is solely responsible for completion of his/her application. The program is not responsible for deficiencies due to the US postal service or third parties in order to complete an applicant's request for supporting documents. Failure to provide complete and accurate information or providing false information will result in immediate withdrawal from admissions consideration. All applicants should be aware that gaining admissions to the Emory Anesthesiologist Assistant Program is highly competitive. No applicants are guaranteed admissions to the program, even when the general requirements are met. |
